Healthcare transitions can be emotional, confusing, and heavy — especially when a loved one is moving into memory care, assisted living, returning home from the hospital, beginning hospice care, or needing a higher level of support.

 

Often the decision may be necessary, but that does not make it easy.  There may be:

  • Feelings of guilt.

  • Second-guessing.

  • Family tension.

  • Questions about what comes next.

And often, caregivers are expected to keep going without anyone stopping to support them.
